Roles of Patient Education Department

I. Patient Education Department - being a resource for staff and patients in the establishment and development of patient/family education activities.

II. Center for Community Health Promotion and Wellness - assess, plan, coordinate, and evaluate all community health promotion activities in collaboration with churches, schools, block associations, senior citizen centers, businesses, community-based organizations. To maintain community relationships through the provision of:

Bi-weekly Cardiac Risk Assessment Screening:
Every Monday (except holidays) at Family Health Service-840 Lefferts Ave. and Every Wednesday at University Hospital of Brooklyn, Suite C-470 Clarkson Ave.
Facilitate with the Department of Social Work free monthly Downstate Healthy Heart Club held on the second Wednesday of each month
Coordinate cardiovascular forums twice a year that is free for the general public to attend
Weekly Free Smoking Cessation Support Group with free Nicotine Replacement Therapy (patches and lozenges)
Free Weekly Free Nicotine Replacement Therapy Program in Suite C every Wednesday
Free Monthly Stroke Club held on the last Friday of each month except December
Free Monthly Brooklyn Diabetes Club of Downstate Medical Center held on the first Wednesday of each month except for July and August
Other community outreach projects (Community Health Fairs, National Heart Month, National Wear Red Day for Women, National Diabetes Alert Day, National Hospital Week, National Diabetes Awareness Month, etc.).
